The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Queens Neighborhood Health, the Bureau of Chronic Disease Prevention and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner also serves as the Agency's Chief Equity Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to be ter address racial health gaps and improve health outcomes for all New Yorkers. The Bureau of Brooklyn Neighborhood Health is seeking a Program Coordinator to support the implementation of the Creating Health Schools and Communities (CHSC) grant funded by the New York State Department of Health. The Program Coordinator will coordinate and implement CHSC strategies designed to increase access to physical activity, support active transportation and Complete Street initiatives, strengthen school and community wellness environments, and advance policy, systems, and environmental changes that support healthier communities in East Brooklyn. The Program Coordinator will have primary responsibility for coordinating CHSC physical activity, active transportation, and built environment activities, while also supporting school-based and other CHSC strategies as required by the grant. The position will work closely with schools, community-based organizations, residents, government agencies, and other CHSC partners to implement grant activities; provide technical assistance; coordinate community and stakeholder engagement; collect and maintain required program data; monitor progress toward grant deliverables; and support required reporting and evaluation. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: - Coordinate implementation and monitoring of assigned CHSC grant strategies, activities, work plan objectives, and deliverables. - Lead CHSC Complete Streets, active transportation, built environment, and physical activity strategies, including community assessments, walking and biking activities, stakeholder engagement, outreach, and related programming. - Convene and facilitate CHSC coalition, partner, and stakeholder meetings related to active transportation, physical activity, school wellness, and community health. - Develop and maintain relationships with schools, community-based organizations, residents, city and state agencies, and other partners necessary to achieve CHSC grant objectives. - Support implementation of CHSC school-based strategies, including school wellness councils, physical activity initiatives, assessments, policy and environmental strategies, presentations, and technical assistance. - Provide technical assistance and capacity-building support to CHSC schools, community organizations, and other participating partners. - Coordinate CHSC community engagement activities, events, site visits, assessments, presentations, and outreach necessary to achieve grant deliverables. - Collect, track, maintain, and review required CHSC programmatic data, including partner engagement, activities, assessments, participation, technical assistance, and progress toward grant objectives. - Ensure timely and accurate entry of required CHSC data into Public Health Partners Connect (PHPC) and other required data systems. - Monitor progress toward assigned CHSC deliverables and identify implementation challenges, gaps, and opportunities for improvement. - Support preparation of CHSC grant reports, work plans, presentations, evaluation activities, documentation, and other materials required by the New York State Department of Health. - Collaborate with CHSC program leadership and team members to ensure grant activities are implemented in accordance with approved work plans, timelines, and funding requirements. - Participate in CHSC community events and activities, including occasional evening and weekend programming when required to meet grant objectives. - Perform additional responsibilities directly related to CHSC implementation, reporting, evaluation, and grant deliverables. PREFERRED SKILLS: - Strong experience in community engagement, public health programming, active transportation, physical activity, built environment initiatives, or related community-based work. - Demonstrated ability to develop and maintain effective relationships with schools, community organizations, residents, government agencies, and other stakeholders. - Experience coordinating programs, community events, coalitions, or multi-partner initiatives. - Strong facilitation and public presentation skills. - Experience providing technical assistance, training, or capacity-building support to community or school-based partners. - Experience collecting, tracking, managing, and reporting programmatic data. - Ability to interpret and communicate public health and neighborhood-level information to diverse audiences. - Strong organizational skills and demonstrated ability to manage multiple projects, deadlines, partners, and deliverables simultaneously. - Excellent written and verbal communication skills. - Ability to work independently while collaborating effectively as part of a multidisciplinary team. - Strong attention to detail and consistent follow-through. - Proficiency with Microsoft Outlook, Word, PowerPoint, and Excel. - Experience with Salesforce, Tableau, PHPC, or similar data and customer relationship management systems is a plus. - Familiarity with East Brooklyn neighborhoods and experience working with communities in Brownsville, Ocean Hill, East New York, and surrounding areas is strongly preferred. - Knowledge of health equity, social determinants of health, community-centered public health practice, and strategies to increase access to physical activity is preferred Why you should work for us: - Loan Forgiveness: As a prospective employee of the City of New York, you may be eligible for federal/state loan forgiveness and repayment assistance programs that lessen your payments or even fully forgive your full balance.
