Develop individualized treatment plans focused on maximizing independence and quality of life. Provide therapeutic interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Assess and recommend adaptive equipment, home modifications, and safety strategies. Train patients, caregivers, and families on techniques that support successful home management. Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals. Complete accurate and timely clinical documentation in accordance with agency standards. Qualifications Graduate of an accredited Occupational Therapy program. Current Oregon Occupational Therapist license. Current CPR certification or ability to obtain. Valid driver's license, reliable transportation, and automobile insurance. Home Health experience preferred but not required. Strong clinical judgment, organizational skills, and communication abilities.
